Abstract

The mandible is one of the facial regions most affected by trauma, due to its prominent position in the lower third of the face, with a relevant increase in cases in recent years. Treatment for fractures aims to restore function, anatomy, and pre-trauma aesthetics. The treatment of facial fractures can be carried out by means of conservative or surgical techniques. Thus, the treatment method should be chosen after careful evaluation of the clinical characteristics of the fracture. This article aims to present a clinical case of a parasymphyseal fracture, mandibular head and coronoid process in a 23-year-old male patient, victim of a motorcycle accident, admitted to the Oral and Maxillofacial Surgery and Traumatology Service of the UFMS University Hospital. On clinical examination, this patient presented pain complaints, stepping in the right parasymphyseal region, and left mandibular condyle and occlusal alteration with anterior open bite. CT scan showed a fracture of the right parasymphyseal, left mandibular head and ipsilateral coronoid process. After diagnosis, treatment consisted of conservative treatment of the mandibular head with Erich bar installation, associated with parasymphyseal and coronoid process reduction and fixation, and maxillomandibular block under general anesthesia to promote occlusal stabilization. The patient remained in maxillomandibular block for 45 days. Thus, the association between the surgical approach and conservative treatment with the Erich bar and maxillomandibular block allowed us to restore the function of mandibular movements, facial aesthetics, and satisfactory occlusion.
