Dental trauma is important issue, which has a high prevalence in children and young people, thus being a public health problem, which generates from small damages to the definitive loss of the dental element, causing a great scare to parents. . In cases of trauma, a correct diagnosis is important for choosing the best treatment, which will often require a long-term follow-up in the search for a favorable prognosis. The objective of this work is to report a case of dental trauma with enamel fracture and extensive dentin rehabilitated with restoration through the direct technique guided by the silicone mold performed after the model wax-up. Patient attended the school clinic of a higher education institution, reporting pain in his broken teeth when he drank ice water. In the anamnesis, he reported having fractured his teeth due to a fall at home during a game. Clinical examination showed an extensive enamel and dentin fracture without pulp involvement of tooth 11 and an enamel fracture with small dentin exposure on tooth 21 and radiographic examination showed no major damage involving pulp, root or bone plate. After evaluating the case, a direct restoration guided by the silicone mold was chosen. Treatment started with pulp protection on element 11 with calcium hydroxide cement and glass ionomer cement, followed by a silicone-guided composite resin restoration protocol. The patient remained under clinical and radiographic follow-up for 45, 90 and 180 days, and the pulp condition of the tooth was preserved, and the performance of the restorative procedure was satisfactory. In this way, the importance of the correct diagnosis is concluded, providing the choice of an effective treatment, through a simple technique that presents some important technical details. In addition to ensuring the maintenance of tooth health through follow-up.
